You must agree to the terms and conditions of SUA membership to continue. Annual cost of membership is $40.00 plus the FHSAA yearly dues.
- I acknowledge that approval of my application WILL NOT constitute an employment agreement between myself and the SUA.
- I certify that I will function as an independent contractor while serving as an SUA registered official.
- I certify that I possess and will maintain a valid Florida driver's license and automobile insurance as required by Florida statutes.
- I agree to abide by all of the SUA's policies and procedures, constitution, and by-laws.
- I agree to abide by the FHSAA regulations governing the conduct of all contest officials at SUA contracted games.
- I agree to register with or retain/renew my membership with the FHSAA.
